How to start a visit

  1. Open the Intermountain Health MyChart App
  2. Select Virtual Visit.
  3. Start a Connect Care Scheduled visit.
  4. After selecting your current location, choose the Connect Care: Physical Therapy practice, and select a provider.

If you have questions about medications or need a prescription, please contact your physician or another qualified medical provider. Connect Care Virtual Physical Therapy offers scheduled video visits with licensed physical therapists. During your visit, your therapist completes a virtual evaluation, develops a personalized plan of care, and guides you through exercises tailored to your needs. Appointments are scheduled for 40 minutes, and you can complete your care from home without traveling to a clinic.
Connect Care Virtual Physical Therapy accepts most insurance plans, with cash‑pay options available. Coverage may vary by plan. Please check with your insurance provider for details.

Connect Care Virtual Physical Therapy treats a wide range of conditions through convenient scheduled video visits, helping patients reduce pain, restore movement, and stay active from home. Our licensed physical therapists commonly work with people experiencing back and neck pain; shoulder conditions such as rotator cuff injuries, impingement, or frozen shoulder; knee pain including ACL, meniscus, and arthritis‑related issues; and hip pain or mobility limitations.

We also treat sports and overuse injuries, provide guided postsurgical rehabilitation, and help manage arthritis, joint pain, tendon and muscle strains, and chronic pain conditions. In addition, Connect Care Virtual Physical Therapy offers pelvic health services for conditions such as pelvic pain, incontinence, and pregnancy‑ or postpartum‑related concerns, as well as balance training and fall prevention.

Yes. Virtual physical therapy has been shown to be as effective as in‑person care for many musculoskeletal conditions and can save time and money. Through a video‑based appointment, you receive personalized education and guided exercises from a licensed physical therapist.
