Accessing your records

The Intermountain health app, powered by MyChart, gives you quick and secure access to a limited set of your health records at no cost.

You can view:

  • History & physicals
  • Discharge summaries
  • Lab results
  • Imaging reports and actual images
  • Emergency Department (ED) notes
  • Progress notes

Download the app or sign in from your browser.

  • Then sign in to your MyChart account and go to Menu > Sharing > Request Records
  • You can request specific visits or documents not available in MyChart
  • Electronic delivery options available

Request full medical records

If you’re looking for records not available in MyChart, we can help you request them in other ways. This includes:

  • A complete copy of your medical record
  • Requests for third-party access (such as insurance or legal representatives)
  • Submitting an amendment or correction to your record

You can request records by:

  • Mail, fax or emailing in an authorization form
  • Visiting a facility in person

Looking to request a copy of a birth certificate?

Though we can provide medical information and records, we cannot provide copies of birth certificates. To obtain copies of a birth certificate, please contact your state's vital records department or health department to request those records:

Medical records from Primary Children’s Hospital

Because Primary Children’s Hospital is a collaborative partnership between Intermountain Health and the University of Utah, where you request your child’s medical records depends on who provided the care.

Want to view records from both systems in one place?

You can link your child’s records from Intermountain and the University of Utah using a MyChart feature called Happy Together.

This allows you to view health information from multiple health systems in one account — including appointments, test results, medications, and more.

How to link your child’s MyChart accounts:

  1. Log in to your account and select your child’s profile
  2. Go to Menu > Link My Accounts
  3. Search for and select University of Utah Health or any other healthcare organization you’d like to connect
  4. Log in using your credentials for that organization

Once linked, you’ll be able to view your child’s information from both systems in a single MyChart experience.

Granting access to friends or family

You can give someone access to your medical information through a MyChart account — this is called proxy access.

  • Adults (18 and older): Grant proxy access anytime by signing in to your MyChart account and selecting Share My Record.
  • Children age 12 & under: A parent or legal guardian can request access by submitting the “Request Access to a Child’s Record” in the MyChart menu.
  • Teens (ages 13–17): Teens can create their own MyChart account and grant access directly to a parent or guardian.

Share your records

Share your records with another provider/clinic/hospital

If you’d like to send your records directly (for example, to another healthcare provider or facility), you can do this securely through the Intermountain Health app, powered by MyChart, following these directions:

How to send records:

  1. Sign in to MyChart
  2. Go to Menu > Sharing > Share My Record
  3. Choose to Download, Print, or Send your record electronically 

For healthcare providers: Intermountain Health participates in Care Everywhere, a secure tool that allows providers to request and receive your records directly — no action needed from you.

Tip: For the fastest processing, ask your provider to request your records directly from Intermountain through their EHR system.

Amendments to medical records

Learn more

  • An amendment is a formal request to alter or create an addendum to a record. It is used in cases where it is not felt that the original documentation accurately reflects the condition, diagnosis, treatment, or other actions relating to health care provided to the patient. Please note that Intermountain Healthcare is unable to change health information in a document but can create an amendment to the record.
  • It is required that the request is submitted in writing.
  • Please be as specific as possible regarding information that is felt to be inaccurate, as well as why it is felt to be inaccurate, to aid in the decision-making process.
  • One or more amendment requests may be made. Each request will be considered separately.
  • Because the original record is a legal document, the disputed wording cannot be changed, but an addendum can be attached to the record for future use.
  • Generally, notification regarding approval of the amendment request will be made within 60 days. If a decision cannot be made within 60 days, you will be notified regarding why there is a delay, and there may be a 30-day extension.
  • A copy of the amended information will be sent to you as soon as it has been completed.
  • You may request that copies of the amended record be sent to other healthcare providers or health insurers you designate. An authorization form will be sent to you with the copy of the amended information and must be sent back within the designated timeframe.
  • If it is determined that an Intermountain Healthcare employee or healthcare provider did not create the disputed information, an amendment to the record cannot be done.
  • The disputed information is in a record that is never used to make healthcare treatment decisions or payment decisions.
  • The information may be a record that the Federal law indicates is not available for you to amend.
  • The author of the information believes the record is accurate and complete as it was originally documented.
  • Generally, notification regarding denial of the amendment request will be made within 60 days. If a decision cannot be made within 60 days, you will be notified regarding why there is a delay, and there may be a 30-day extension.
  • A letter will be sent with an explanation of why the amendment request was denied.
  • You will be provided with information regarding where a complaint can be made, both at Intermountain Healthcare and the Federal agency that handles these complaints.
  • You will have one of three options:
    • Option 1: Do nothing.
    • Option 2: You can request that a copy of a summary of your amendment request and Intermountain Healthcare's response be sent every time a copy of the disputed information is released.
    • Option 3: You can submit a written statement of disagreement.
  • If a written statement of disagreement is submitted, it will be scanned into the medical record and maintained with the information in question. It will be released any time the information in question is released.

NOTICE on intent to destroy medical records

Healthcare facilities must retain patient medical records in accordance with federal and state regulations. Once the retention regulations are met, medical records may be destroyed following organizational policy. Beginning 4/1/2026, we will begin the destruction of Lutheran Medical Center, St. Joseph Hospital, and Platte Valley Hospital older medical records.  You may request your medical record prior to destruction.

Paper medical records for adults that have not been seen in the hospital since 12/31/2015 shall be destroyed.

Paper medical records for minors that have not been seen in the hospital since 12/31/1997 shall be destroyed.
